How to turn in forms

I. Immunization Records

Students, interns, faculty and others who will be on the OSH campus for eight hours or more are required to have had a seasonal influenza vaccination and a tuberculosis assessment – whether you are inside or outside the secure perimeter.

Documentation of immunizations and/or tests must be sent to the Infection Prevention Office via U.S. mail, fax or email:

You may drop off the forms at the Salem Campus Communications Center or the Junction City campus Reception window. Clearly mark the envelopes with your contact person's name (see table below).
