Living Well and other workshops based on the Stanford Chronic Disease Self-Management Program (CDSMP) model are available in many counties in Oregon. These include Tomando Control de su Salud, the Spanish-language version of Living Well, the Diabetes Self-Management Program, and the Positive Self-Management Program (PSMP) for people living with HIV/AIDS.

Program Length
A six-week program that meets 2½ hours each week. (Note: Positive Self-Management runs for seven weeks.)
Who Attends
People with different chronic conditions and their support people (family or friends).
Leaders
Two trained leaders at each location. At least one of them has a chronic condition.
Cost
Some sites may charge a small fee to cover the cost of the course materials, which includes a book.
