Investigative Guidelines for HIV

Newly diagnosed cases of HIV are reportable in Oregon by county via laboratory data. Forms and information on reporting criteria are available below:

These guidelines represent the first major revision since 2007. Draft guidelines were revised earlier in the year based on review and suggestions by interested parties representing local health authorities. The HIV Subcommittee of CLHO reviewed the guidelines during July. A log of revisions is available from the HIV Surveillance Program (971-673-0153) upon request. Important additions and changes to the guidelines include:

  • Confirmed cases require laboratory confirmation with either a positive antibody test followed by a positive confirmatory test of a different type, or detection of HIV antigen.
  • Suspect cases have been defined for persons <18 months and persons ≥18 months. In general, among adults, suspect cases are cases for which an initial laboratory test for HIV antibodies is positive for HIV without confirmation. Among children ≤18 months, a suspect case is one for which laboratory confirmation is lacking after a single antigen test or physician report suggest HIV infection.
  • A new classification system for staging HIV infection has been introduced. Stage 0 represents recently infected cases; Stage 1 represents asymptomatic, non-acute cases; Stage 2 represents non-acute infection of intermediate duration where CD4 count is ≥200 cells/mm3 but <500 cells/mm3; Stage 3 represents cases formerly designated as AIDS, for which an opportunistic illness was present at diagnosis or CD4 count was <200 cells/mm3.
  • Procedures for investigation of newly reported cases by local public health authorities have been updated.
  • Resources for referral and linkage to care for new cases have been updated.
  • Local health authorities are encouraged to enter information about newly reported cases directly into, Orpheus, the statewide disease reporting database. New case report forms and instructions have been created to be used when direct entry into Orpheus isn't feasible or to facilitate subsequent entry of case data into Orpheus.
