The 2012 Living Well County Data Fact Sheets are available for only those counties that held at least one workshop in 2012. Other counties that offered a workshop in 2011 or earlier but not in 2012 will continue to have access to their 2012 Living Well County Data Fact Sheets. Please note that the length of a County Fact Sheet varies by the number of participants that have attended a workshop in each county.

The Oregon Department of Human Services asks your assistance in collecting information about where Living Well programs are being offered across Oregon and who is participating. This information helps track statewide successes and identifies additional training, funding, and other support needs.

CD Summary Newsletters: Living Well
The CD Summary is a publication of the Oregon Health Authority Public Health Division, and is distributed every two weeks. The intended audience is licensed health care providers; public health and health care agencies; media representatives; medical laboratories; hospitals; and other individuals and institutions with an interest in epidemiology and public health interests.

Healthy Aging in Oregon Counties - This 2009 report provides an overview of issues impacting the health of older Oregonians, and to help aging, public health, and other community organizations develop effective strategies to help older Oregonians age safely, remaining healthy and independent as long as possible.
Living Well Impact Report (pdf) - This report, written by Dr. Viktor Bovbjerg and Mrs. Sarah Jane Kingston from Oregon State University, provides estimates of the impact of Living Well between 2005 and 2009 as well as projected impact should the program reach 5% of Oregonians with a chronic condition.

Living Well Cost Calculator (xlsm) NOTE: "Right-click" and download this Excel file for use. The file contains macros and is intended to be opened with Excel. This interactive tool helps organizations better understand and mange the costs of offering CDSMP. You can use the calculator to estimate "per participant" and "per workshop" costs, as well as total program expenses.
