Health System Transformation and SBHCs
Oregon has prioritized the conversion of our health care system to a model that focuses on ensuring a healthy population through person-centered care in a cost-effective manner through the development of Coordinated Care Organizations (CCOs). School-Based Health Centers (SBHCs) are natural partners in the health care reform movement to help ensure a healthy population of children and adolescents.
The SBHC State Program Office (SPO) strongly encourages all SBHCs to become state-recognized as a Patient-Centered Primary Care Home (PCPCH). Coordinated Care Organizations are required to include recognized primary care homes in their networks of care to the maximum extent feasible.
