CEU Reporting Process

Effective January 1, 2016, DWS began a new CEU reporting process to improve renewal efficiency. Applicants for renewal, signed the CEU Self Certification form indicating they have obtained the required number of CEUs within the preceding certification period as required, and listed the specific classes or trainings they attended. 
Operators who are selected for an audit will receive notice by postcard after March 15th. Selected operators will then have until April 15th to submit copies of their training documentation for review. Acceptable documentation includes copies of agendas, attendance and completion certificates, along with receipts for registration and payment at short schools and similar training events.

Instructions

You will have already submitted your completed operator certification renewal including the CEU Self Certification form (pdf). If you receive an audit postcard in the mail complete the following steps:

  1. By April 15 send in copies (not the originals) of the certificates for the specific training identified on your CEU Self Certification form (pdf) during renewal.
  2. Mail copies of training certificates to: 800 NE Oregon Street, Suite 640; Portland, OR 97232
  3. Operator Certification staff will validate your training documents and will contact you if there are any questions.
  4. To receive your audit results please provide an email address when you submit your audit documents.
