Local Health Departments

Order Certificate of Immunization Status (CIS)
Update mailing lists in IRIS
July-August

Schools & Child Care

Make sure you have immunization records for all enrolling students.
Train new school/children’s facility staff.
Confirm # of reporting packets you will need
Develop a cover letter that outlines the primary review process
Distribute packets
Schedule immunization clinics for exclusion time
September-November Distribute immunization rates 30 days after the start of school 
Evaluate Certificate of Immunization Status (CIS) forms and determine whether students are missing immunizations or other information
Continue to have parents add updated immunization information on CIS forms
Confirm that schools/facilities received materials
Make necessary updates to contact information in IRIS
Write/update a cover letter to be mailed with exclusion letters
December Review forms and information in reporting packet
Call schools and children’s facilities who have not sent reports in by 2nd week of January. Due by January 8 Primary Review Summary (PRS) Sections A and B, and copies of the Certificate of Immunization Status (CIS) forms of children who have incomplete or insufficient immunization records, or have medical exemptions needing review are due to the local health department.
Example:

Local health departments review records and issue exclusion orders. January 18-Febuary 6
Copies of the exclusion orders are mailed to parents and schools/children’s facilities. Mailed by February 7
February 21 EXCLUSION DAY: Send home children whose exclusion orders are not canceled by the time class starts on this date. There are no exceptions to exclusion on this date.
Due by March 5 Sections D, E, F and/or G of the PRS are due to the local health department.
Example:

The County immunization Status Report is completed by the local health department and due to the Oregon Immunization Program. Due by March 16
Due by March 23 Distribute school and county immunization rates.
Send county immunization rates to all schools and child care facilities Due by May 15
