Driving Under the Influence of Intoxicants (DUII): Treatment
DUII Treatment Certificate
Individuals who are convicted of a DUII will need to obtain a DUII Treatment Completion Certificate. A DUII Treatment Completion Certificate is one of the legal documents that the Department of Motor Vehicles (DMV) requires for driving privilege reinstatement. DUII Treatment Completion Certificates are a four part DMV form (form 735-6821) and include a unique identification number and Oregon's state seal. Other documents may have been presented to an individual by their treatment provider at the time of treatment completion, but these will not satisfy DMV requirements. DUII Treatment Completion Certificates are only issued by Oregon Health Authority's Addictions and Mental Health (AMH) Division approved providers or directly from the AMH Division.
