Self-Monitoring of Blood Glucose for Type 1 & 2 Diabetes


As a part of the coverage guidance monitoring process, the HERC decided on 1/14/2016 (see Appendix D) to reaffirm the existing coverage guidance and reconsider the need to update the topic during the regular two-year review cycle.


HERC revised and approved the Coverage Guidance and Prioritized List changes for the Prioritized List.


HERC leadership requested that alternative language for the draft coverage guidance box be posted in advance of the 12/5/2013 HERC meeting. Leadership Alternative


HERC discussed the draft coverage guidance and will continue discussion 12/5/2013.


A revision to the draft coverage guidance has been posted to correct a clerical error. The version posted 9/26/13 stated the incorrect strength of recommendation for patients with type 2 diabetes not requiring multiple daily insulin injections or meeting exceptions. The correct recommendation strength is “strong recommendation.”  Draft Coverage Guidance

VbBS met 5/9/13 and accepted the recommendations in the meeting materials for a new guideline note for the Prioritized List to be based on the draft coverage guidance HTAS is recommending to HERC. These guideline recommendations for Oregon Health Plan coverage differ from the proposed coverage guidance in that only 50 rather than 100 test strips per 90 days would be covered for type 2 diabetes patients who do not require multiple daily injections but have complicating factors in the management of their disease. The HERC will review the recommendation at its meeting 10/10/13.
