Umpqua clinic to offer expanded care for highest-need clients

June 3, 2013 (Roseburg) – For Linda Mullins, it’s about putting the human body back together.

“When we split apart physical health, mental health and oral health, we created problems,” says the chief executive officer of Umpqua Community Health Center.

Now the Umpqua Health Alliance coordinated care organization (CCO) is bringing all health services under a single roof at the center’s Roseburg clinic. The “expanded care clinic” will serve the county’s Oregon Health Plan members who are the highest users of health services - people with complex health needs who have not been well-served by a fragmented health system.

To find the members who could benefit most from the clinic’s services, the CCO looked at pharmacy, emergency room and claims data to identify up to 100 people with multiple chronic illnesses that included undertreated oral health and mental health issues.

The clinic opened its doors to its first patient in May. Clinic staff will track 17 indicators of members’ health every month and make quarterly reports to the CCO board.
