What information is needed?
To get on the list, we just need the following information about you and anyone in your household ages 19 and older who wants to be added to the list:
Name
Mailing address
Date of birth
There are four easy ways to sign up for the drawing list
Online
Fill out the request form
Call
1-800-699-9075 or TTY 711, Monday through Friday, 7 a.m. to 6 p.m. The call will take from 10 to 20 minutes.
In person
Drop off a completed drawing entry request at any Department of Human Services field office
(Staff may fax request forms directly to the OHP Processing Center.)
Print a copy of the request form:
