Reportable Diseases and Forms

Veterinary Disease Reporting Poster

On this page:

The Acute and Communicable Disease Prevention Program gathers information provided by veterinary laboratories to track diseases that are transmitted from animals to humans. Reports include zoonotic diseases such as rabies, leptospirosis, campylobacteriosis, parasites, and tick-borne diseases.

Veterinarians also are welcome to complete the reporting form for zoonotic diseases and diseases of veterinary importance (pdf) for cases diagnosed in clinical practice.

View the comprehensive list of zoonotic diseases in the U.S. (pdf).
