The HIV Medical Case Management Program addresses the needs of persons with HIV disease by funding both county and regional-based case management and support services that support access to and retention in medical care

County Based Service Model

The county based model of HIV case management was implemented in 1999 as a formal and professional service that links clients with HIV/AIDS and multiple service needs to a continuum of health and social service care systems.

The HIV Medical Case Management Task Force convenes every two years to review and modify these documents.

The following Standards of Service and forms are utilized by all county based case management service providers and are required by contract.

Regional Based Service Model

The regional based model of HIV care coordination was implemented in 2009 as an alternative to county based services and is a formal and professional service that links clients with HIV/AIDS and multiple service needs to a continuum of health and social service care systems.

The HIV Medical Care Coordination Task Force convenes every two years to review and modify these documents.
