DESCHUTES COUNTY

DESCHUTES COUNTY - Public Health Nurse II - Psychiatric - Bend, Oregon

Deschutes County is currently accepting applications for three Public Health Nurse II's - Psychiatric. These positions are part of an innovative, new, 2-year federal grant project that will focus on providing out-patient services to underserved individuals in our community with identified mental health and/or substance use disorders.

  • Incumbents will provide a variety of registered nursing duties working with adults and/or children with mental health and/or substance use diagnoses. Responsibilities will include assisting clients with daily and weekly medications including injections; and providing a variety of behavioral health services including case management, skills training etc. as appropriate.
  • Incumbent will assist with prescription refills, monitor side effects and provide education. These services will be primarily office based, but may include services in client's home or within the community. Incumbent may be assigned to more than one clinic location.
  • Incumbent will work with a variety of clients including those with serious mental illness, and will coordinate and consult with behavioral health, medical and other staff and community partners on behalf of clients.
  • Successful candidate will work as part of a multidisciplinary team and should be accomplished in collaboration with other members of the treatment team and able to develop and maintain positive relationships with community partners.

MARION COUNTY

Public Health Nurse 3 - Marion County Health Department - Salem, OR

This position provides direct case management service for women and their children through Babies First, CaCoon, and Maternity Case Management Programs. To include:
  • Serves as CaCoon Coordinator for Marion County Health Department (a CaCoon contract requirement).
  • Coordinates training & day to day activities of the Public Health Worker 3, including regular consultation & oversight of activities.
  • Conducts home visits, performs comprehensive holistic assessments of the client, including physical, hearing, vision, developmental screening and other screenings, and assessment of psychosocial needs. Obtains heights, weights, and blood pressures as appropriate.
  • Provides information and supports client access to service; advocates for client need.
  • Provides case management coordination of services to prevent duplication, promote continuity of care, & provides integration of client services to meet client & community needs.
  • Reviews, evaluates, & modifies care coordination with family & monitors progress. Develops and implements Shared Plans of Care with community partners and families according to contract guidelines.
  • Records in medical records.
  • Participates in planning & implementation of activities SCHN (Services for Children with Special Health Needs) staff.
  • Develops & provides consultation in creation of informational materials providing information about disabilities & CaCoon role in serving this population.
  • Collaborates with other community service providers, including physicians, therapists, pharmacists, home health agencies, schools, hospitals & community/state agencies.
  • Drives motor vehicle to home visits.
  • Maintains productivity standard of 6 visits per week to clients on her caseload.
  • Caseload range of 32-36.

This position performs administrative duties to support the management of services provided within the UCo Health clinics. Develops and maintains positive and cooperative working relationships with public health team, community partners, clients, families, providers and state and local agencies. Plans, organizes, and manages the development, implementation, and on-going operation of highly professional and specialized programs, such as immunizations and family planning. Initiates and implements strategic and management activities, with the consent and support of the Public Health Director, to maintain a varied and complex array of services to meet community need. Manages and administers revenue and expenses associated with services; plans, coordinates, and supervises health services for assigned health programs. Provides skilled professional nursing care; provides community and regional leadership and support. Understands and interprets federal, state and local rules, regulations, code and ordinances. Provides backup for intake, service coordination and quality assurance duties.

REQUIREMENTS:

  • Bachelor's Degree in Nursing from an approved school of nursing with license in good standing, required.
  • Master's Degree in Nursing or closely related field, preferred, which includes two (2) years of supervisory experience
  • Experience in communicable disease, home visitation, community health and/or maternal/child health services preferred
  • Maintain confidentiality
  • Ability to accept and utilize supervision and lead by example
  • Ability to work with people of all socioeconomic culture groups
  • Current valid Oregon driver's license.

UMATILLA COUNTY - EASTERN OREGON PDO GRANT COORDINATOR – PUBLIC HEALTH – PENDLETON, OR

The Eastern Oregon Prescription Drug Overdose Prevention (PDO) coordinator will coordinate opioid overdose prevention efforts in Umatilla, Union, Baker and Malheur counties in accordance with Oregon Health Authority (OHA) regional pain guidance programming. The PDO Coordinator will be housed at Umatilla County Public Health in Pendleton, Oregon. Frequent travel to Union, Baker and Malheur counties will be necessary.
