Oregon offers a variety of self-management programs to promote taking control of your life and living healthier.

Living Well With Chronic Conditions and other Stanford Self-Management Programs  

Living Well and other workshops based on the Stanford Chronic Disease Self-Management Program model are available in many counties in Oregon. These include Tomando Control de su Salud, the Spanish-language version of Living Well, the Diabetes Self-Management Program, and the Positive Self-Management Program for people living with HIV/AIDS.

Workshops are offered in most communities across Oregon. 

Oregon Tobacco Quit Line

The Oregon Tobacco Quit Line is a free program offering tips, information, and one-on-one telephone counseling to anyone looking to quit tobacco or help someone quit.  Available to all Oregonians regardless of income or insurance status, the Quit Line is staffed by real people who are friendly and non-judgmental.

Quit coaches will help you make your own plan to fit your own life. No pressure, no guilt. Coaches can even help you figure out if you are eligible for free nicotine gum or the patch.

Open 7 days a week, 4:00 am to 12:00 am. Coaching is available in many languages.

Oregon Arthritis Program

Walk With Ease Program

The Arthritis Foundation Walk With Ease (WWE) Program is a community-based physical activity and self-management education program. It can be done by individuals using the WWE guidebook on their own, or by groups led by trained leaders. Both the individual and group formats are set up as a structured six- or nine-week program. While walking is the central activity, WWE is a multi-component program that also includes health education, stretching and strengthening exercises, and motivational strategies. Group sessions include socialization time, pre-walk informational lecturettes, warm up and cool downs, and a 10-35 minute walking period.  

The WWE classes are held in various locations throughout the state.

EnhanceFitness Program

The EnhanceFitness (EF) program is a multi-component group exercise program designed to increase strength, improve flexibility and balance, boost activity levels, and elevate mood. EF includes a five-minute warm-up, 20-minute aerobics workout, five-minute cool-down, 20-minute strength training and 10-minute stretching. EF is offered to groups of 10 to 25 participants as one-hour classes that meet three times per week on an ongoing basis.  

The EF classes are offered by YMCA in several Oregon counties.

National Diabetes Prevention Program

The National Diabetes Prevention Program is an in-person or virtual evidence-based lifestyle change program for preventing type 2 diabetes. The year-long program helps participants learn ways to incorporate healthier eating and moderate physical activity, as well as problem-solving, stress-reduction and coping skills into their daily lives. Participants meet with a trained lifestyle coach and a small group of people who are making lifestyle changes to prevent diabetes. Sessions are weekly for the first six months and then at least monthly for the next six months. This proven program can help people with prediabetes and/or at risk for type 2 diabetes make achievable and realistic lifestyle changes and cut their risk of developing type 2 diabetes by 58 percent.

Falls Prevention for Older Adults

Tai Chi: Moving for Better Balance (TCMBB)

Tai Chi: Moving for Better Balance is an eight-form version of Tai Chi that was designed to improve strength and balance while reducing fear and risk of falls. Participants attend one hour classes 2-3 times per week for three months.
