Monthly request for Housing Assistance Payment (HAP) must be received by the 10th calendar day of each month.  Property submission requires the following: 
  • Original signed paper HAP consisting of:
    • HUD-52670
    • HUD-52670-A part 1
    • HUD-52670-A part 3 only if HAP includes adjustments
    • HUD-52670-A part 5 only if approved/adjusted Special Claims are requested for payment
    • HUD-52670-A part 4 only if HAP includes Miscellaneous adjustments
  • Electronic Transmissions consisting of:
    • MAT30 (voucher)
    • Tenant data (if applicable)
