APPLICATION AND REGISTRATION FEES
  • Fees are set by law. No exceptions are made to the current fees.
  • Read eligibility requirements carefully. Once fees are submitted, they will not be refunded, credited, transferred or prorated.
  • The application processing fee is required before the Board staff begins the processing of the application. Once received, the application form and fees are valid for a period of one year from the date received. If licensure is not obtained within that one-year period, a new application and processing fee must be submitted as if filing for the first time. The applicant is expected to meet current requirements.
  • The application fee does not include the Initial Registration fee, which will be assessed once the application is complete. Registration fees are based on license type and license status. Licenses will be approved once the Initial Registration fee is received by the Board.
  • Applicants who are licensed in the second year of the biennium will be assessed the registration fees for only one year.
  • Do not submit Special Purpose Examination (SPEX) fees to the Oregon Medical Board.
  • Fees requested by any other agencies to provide verification to the Oregon Medical Board will be the responsibility of the applicant.
  • Additional documents that may be required as part of the licensure process such as court documents, hospital records, etc. will be provided by the applicant. OMB will not be responsible for any cost incurred.
