The Oregon Medical Board provides the most frequently requested public information in standard formats, see the "By Request" section above.
For information not included in these standard reports, submit a written request to the Oregon Medical Board. Be sure to identify as specifically as possible the type of record(s), subject matter, approximate date(s), names of persons involved, and name and contact information for the person requesting the public information. The Oregon Medical Board will notify the requestor of the required charges within 10 business days.

By calling between 8:00 AM - 12:00 PM and 1:00 PM - 4:30 PM, Pacific Time a person can obtain the following information about any health care provider licensed by the Board: license number, date first licensed in Oregon, expiration date, license status, whether or not a licensee is in good standing with the Board, mailing address, business address, business phone number, year of birth, specialty, and location of patient records. Limited to 3 names per phone call.
