DNA Submissions for Missing Persons and Unidentified Human Remains Cases

Please follow these guidelines when submitting DNA for Missing Persons and Unidentified Human Remains to the laboratory:

·         Adhere to your agency’s protocol regarding the time elapsed from missing person report to sample submission (usually 30 days after a missing person has been reported as missing).

·         Fill out an OSP Request for Service (Form 49) and clearly state the full name of the family member sample donor, AS WELL AS the full name of the missing person.

o   Mark the family member on the Form 49 as “mentioned”.

o   Requested Service should state “DNA analysis as a standard from family member” for Family Reference Samples.

o   Mark the missing person on the Form 49 as “victim”.

o   Requested Service should state “DNA analysis as a direct missing person sample” for samples such as toothbrushes from the missing person.

·         You may use the UNT submission paperwork for additional information, but the Form 49 must still be included.

·         Include a police report documenting the general scenario for the missing person case.

·         NCIC numbers are helpful but not required for sample submission.

·         NCMEC numbers are helpful but not required for sample submission.
