Two people were injured, one seriously, in a collision Thursday morning on Highway 99W just south of Monmouth in Polk County. Oregon State Police (OSP) is asking for contact from any witnesses who have not spoken to the lead investigator.
On January 16, 2014 at approximately 6:33 a.m., fire, police and medical personnel responded to a reported injury collision on Highway 99W near Stapleton Road south of Monmouth.
Preliminary investigation indicates a 1989 Mazda MX6 driven by HYATT WALBERG, age 29, from Corvallis, was traveling northbound when it crossed the centerline and collided into the left side of a 2004 Freightliner truck and the first of two semi-trailers loaded with lumber. The left side diesel tank was ripped off and spilled diesel across the highway. The truck caught fire and was burned to the ground while firefighters from Dallas and Polk County Fire District No. 1 worked to prevent further spreading.
WALBERG was seriously injured and extricated by firefighters. She was transported by ambulance to Good Samaritan Regional Medical Center in Corvallis.
The truck's driver, DWAINE MERCHANT, age 65, from Tillamook, received minor injury and was transported to an area hospital.
OSP troopers from Salem and McMinnville offices responded to the scene. OSP was assisted by Polk County Sheriff's Office and ODOT. The highway was closed about 3 hours.
