The continued investigation by Oregon State Police (OSP) Collision Reconstructionists into an August 26, 2013 double fatal traffic crash on Crow Road south of Highway 126W confirmed that a preliminary determination for the manner of the crash was incorrect. OSP is correcting the information in this update news release.
On August 26, 2013 at approximately 9:20 p.m., a head-on traffic crash involving two vehicles occurred on Crow Road near milepost 2. The crash resulted in the deaths of the only occupants of both vehicles identified as SHAYLINN PEARL MANCHESTER, age 21, from Veneta, and ZACHARY JACOB MOITOZA, age 29, from Eugene. MANCHESTER was using safety restraints and MOITOZA was not using safety restraints.
The preliminary investigation at the scene that night led OSP to release information that MANCHESTER's vehicle crossed the centerline and collided head-on with the vehicle driven by MOITOZA.
OSP Collision Reconstructionists are required to submit reports on fatal crash investigations involving two or more vehicles. They are also consulted on any crash where a question exists for which their expertise would benefit the investigation.
As a result of the review and investigation by OSP Collision Reconstructionists in this incident, the manner of this crash was confirmed that MOITOZA's vehicle caused the crash by crossing the centerline and colliding with MANCHESTER's vehicle.
Toxicology test results have not been completed to help determine if there were other contributing factors.
OSP notified representatives for both families of the corrected finding related to the investigation.
