Oregon State Police (OSP) troopers are continuing the investigation into a Tuesday afternoon double fatal traffic crash that resulted in the death of a Tillamook-area mother and her 8-year old daughter when their vehicle crossed the centerline and collided head-on with a commercial truck on Highway 6 about six miles east of Tillamook.  The victims' names are being released in this update.
 
On February 8, 2011 at approximately 3:21 p.m. a 2007 Cadillac four-door driven by WENDY LYNN MIZEE, age 33, from Tillamook, was westbound on Highway 6 near milepost 6 negotiating a right curve.  For an unknown reason, the Cadillac crossed the centerline and collided head-on with a 2003 International truck driven by WILLARD T. RICHARDS, age 65, from Salem.  After impact the Cadillac rotated around and both vehicles came to rest in the westbound lane facing east.
 
WENDY MIZEE and her 8-year old daughter, SHELBY MIZEE, were pronounced deceased at the scene.  The mother was using safety restraints and her daughter was seated in the right rear seat position in a booster seat.  The car's airbags deployed.
 
RICHARDS was transported by ambulance to Tillamook County General Hospital with minor injuries.  He was using safety restraints.
 
Highway 6 was closed at the scene until 8:00 p.m. during the investigation
 
OSP troopers and collision reconstructionists from the Tillamook, Newport and Astoria offices responded to investigate.  They were assisted at the scene by Tillamook County Sheriff's Office, Tillamook Police Department, Tillamook Fire Department, Tillamook County District Attorney's Office, Tillamook County Medical Examiner, and ODOT.
