Pursuant to ORS 675.320, all licensed occupational therapists and occupational therapy assistants must notify the board within 30 days of any change of address.
This form is intended for licensed OT's and OTA's who hold an Oregon License as well as current applicants for licensure.
Please keep in mind that the Board must have a physical address on file for its licensees at all times. If you wish to use a PO Box address, please make sure that you also update the physical address on file as well.
