The following forms should be completed and turned in at the time of the interview. Please note that a reference and criminal background check will only be conducted for the top candidates. You will need to download, print, and complete the following:
Background Check Form
Authorization to Access Criminal History Forms (2 forms to complete): complete, sign and seal in a envelope with your name clearly printed on the outside. Also attached (three pages) is informational sheets for you to retain.
Reference Check Form
Reference Release Form: complete, sign and turn in at the interview (no need to be enclosed in an envelope).

We are located just off the Haines I-5 freeway exit or south of 99W on 68th Parkway.
We share our driveway entrance with Comcast and our lobby is on the backside facing the parking lot (we are a three story brick building). Information for interviews at our 72nd Ave Location:

We are south of the 217 Freeway off the 72nd exit.
This location is leased space and our name is not on the building. The building sign does list companies including "OptiLink" and "Fullhart Insurance". Parking is located on either side of the building. Our work space is secured. Go to the second floor and wait by the door (chairs provided) and someone will come get you for the interview.

We are located in the State Archives Building. Located on Summer Street between Union and D Street. Metered parking is available on Summer and D streets and on the Archives property. You will need to check in with reception on the main floor before proceeding to the PERS office reception desk on the second floor. map....
Note: All of our interviews are conducted in a panel format. You will be meeting with two to four employees of PERS, including the Manager of the section. You will be asked a series of questions that you will need to respond to. All members of the panel will have reviewed your application materials that you submitted, so you do not need to provide additional copies at the time of the interview.
