| What You Can Do |

PERS provides all online forms and publications in .pdf format. To view them, you must have the most recent version of Adobe Reader®. Click here to download Adobe Reader®. Many forms are designed to be completed on line. These "fill-in-able" forms may look slightly different when printed. You can fill out most forms on your computer. Simply use the TAB key to move from one field to the next. You can also use your mouse to move from field to field. To do this, position the cursor in the field, and click your mouse. Print out the form before you fill it in on line to make sure your printer has the capability of printing the form correctly. Each form has its own instructions. Make sure you read each form completely, follow all instructions, and sign and date where indicated. Incorrectly completed or unsigned forms will cause a delay in processing your information.
