PERS Health Insurance Program (PHIP) offers an annual plan change period from October 1 to November 15. Information about making changes is mailed in September. During this time, you can change your medical plan to any plan that is available within your residing service area.
Plan changes made during this period become effective January 1. This annual plan change period is not an opportunity to add dental coverage or dependents. If you are enrolled and do not want to change plans, no paperwork is required. However, if you want to make a change, you must fill out both a Termination Form for the plan you are ending and an Enrollment Request Form for the new coverage. Submit both forms to PHIP before the November 15 deadline. You may obtain forms online at www.pershealth.com
or by calling PHIP Customer Service at 800-786-7377. If you do not submit a change during this period, you will be unable to change your enrollment midyear, unless you experience a family status change or new enrollment opportunity.
There are no further sessions scheduled for 2012. The schedule for 2013 will be posted in the near future, so check back periodically.
