If a form does not print as you see it on the screen, we will not be able to process it.
Some members experience difficulty printing the forms correctly when using a Macintosh (Apple) with the Safari browser. If you are experiencing this problem, downloading the free Internet browser Firefox and using it to access and print PERS forms may resolve this issue.

Also, if you are using a Mac (Apple), the PDF viewer does not support some PERS forms. You will need to make Adobe Acrobat or the Reader the default program for opening PDFs.
1. Right click any PDF file on your system.
2. Select "Open with"
3. Select "Other" (the finder opens)
4. Check the box "Always open with" and then select Acrobat or Reader.
