| Quarterly OUS2 Revenue Reporting and Payment Schedule |
|
The schedule for the quarterly OUS2 revenue reports and contribution payments is shown below for recovery quarters through the end of the current year. The reports and payments are due by the dates listed; reports not received by the due date are subject to a late form fee. Payments not received by the due dates are overdue and subject to a late payment penalty and interest. The timely filing of reports and payments is required to retain the telecommunication service provider certification. Failure to file timely reports and payments may result in cancellation of certification in the State of Oregon.
Report forms will be mailed within two-four weeks of the report due date to the contact person reported on the OUS1.

| Monthly OUS3 Line-Count Reporting and Payment Schedule |
|
Telecommunications service providers eligible to receive OUS support payments are required to file monthly line count reports to receive the benefits. The schedule for the monthly OUS3 line count reports and support payments to eligible local exchange carriers is shown below by Report Month through the end of 2011. The reports are due on or before the dates shown.
