District Misconduct Report Forms
 
Complaint forms may be used by anyone (educator, parent, etc.) who wishes to file a complaint concerning an Oregon educator.
 
Per ORS342.176(1)(b), the TSPC Commission may delay the investigation of any public complaint until such time that the complainant provides sufficient documentation to TSPC demonstrating attempts were made to resolve the complaint through the complaint process of the school district that employs the educator. Please review the Public Complaint Guide for further information.   
 
Members of the Public:
  1. Review the Public Complaint Guide for important rules and instructions.
  2. Complete the Public Complaint Form.
  3. Submit the form to: complaints.tspc@oregon.gov.
School District Officials:
  1. Review the School District Misconduct Reporting Guide for important rules and instructions.
  2. Complete the School District Misconduct Report Form.
  3. Submit the form to: complaints.tspc@oregon.gov.
