General Instructions
 
·         These instructions pertain ONLY to completing the C-1 application. 
 
·         The C-1 application form must be completed in its entirety.
 
·        All applicants can either pay by check or money order made payable to “TSPC” and enclosed with this application, OR pay by credit card, by calling (503) 378-3586.  (We do not accept American Express.) 
 
·        Review of your materials will not begin until both the C-1 application and payment are received.

Submitting Application
 
·         This application can NOT be submitted electronically at this time.
 
·         YOU MUST PRINT THIS FORM AND MAIL IT TO TSPC.
o    You may fill it in electronically before printing, or
o    You may print it first, and then complete by hand.
 
·         Once you have completed the form, mail it to our office at:
