Out-of-State Provider Application

Providers in other states must apply and be approved to serve Oregon children and youth in foster care. The Children’s Care Licensing Program in DHS researches the licensing history of potential providers located outside of Oregon, reviews applications and conducts facility visits as part of the process. Regulatory standards are substantively the same for in-state and out-of-state providers.

Out-of-State Provider Statement of Work​Contracts with providers are youth-specific and are tailored to meet the unique treatment needs of each youth receiving services outside of Oregon. The Statement of Work contains the standard requirements all providers must meet.
