Do not use this form for general DMV business or regulation related matters. The Business Regulation unit within DMV provides regulatory related services for vehicle appraisers, dealers, dismantlers, RV show operators, transporters, snowmobile instructors, and driving school operators and instructors. They do not handle general business operations for DMV or manage the organization's compliance with state and federal regulations.
