How to order

  • Register for an In-Person Appointment:

    Available timeslots will be limited to allow for the Center for Health Statistics to follow safety protocols that include adequate social distancing and thorough cleaning of the Public Counters area and equipment.

  • Address:
    Those who have scheduled an appointment should check in ten minutes early at the first floor of The Portland State Office Building, 800 NE Oregon Street.

  • Cost:
    There is a $28.00 nonrefundable fee for the first record, which includes a $3.00 security fee for applicant identity verification and a $25.00 fee for each additional copy of the same record ordered at the same time. Births records from 1903-2007 have an additional $5 charge for a full image birth certificate. Additional records requested as part of a group order will have a $25.00 nonrefundable fee.

    All major company credit or debit cards, cash, money orders, and EFT for personal checks accepted.

    Appointments are scheduled for 30 minutes. Customers will enter their order at a self-service kiosk. Some full image records will not be available the same day but will be mailed or made available for pickup the next work day. Amendments are not completed while you wait.

Scheduled appointments for customers applying in person will use a self-service kiosk to place their order. The identity of customers will be checked using a questionnaire based on public records generated by LexisNexis. To generate this questionnaire, you will be asked to enter your Social Security Number (SSN) as part of the application process. Your SSN and answers to the quiz will not be stored with your application information.

Persons ordering must show a valid ID or alternative identifying documents. If you cannot provide information or documents, we recommend that you have another eligible family member order your record for you. The person who comes to the office to order the record must be eligible to order it.

The non-refundable fee for one record search is $28.00 at the public counter. This fee includes a $3.00 identity authentication fee in addition to the $25.00 record search fee. Additional copies of the same record ordered at the same time are $25.00. For long form, full image birth certificates, there is an additional $5 fee per certificate.

Payment can be made by credit card, debit card, cash or money order. If a personal or business check is used, it must be accepted by the electronic funds transfer software (EFT). A credit union ATM is available in the building. Once an order is submitted and a record search begins, no cancellations or refunds can be made for the first record search fee.
