Clackamas County- The Board revoked Tony D. Klein's registered nurse license (201040837RN) for being convicted of crimes with a demonstrable relationship to the practice of nursing. He resides in Estacada, Ore.
Coos County- The Board reprimanded the registered nurse license of Geanna M. Berrier (200942111RN) for falsifying data and documenting nursing practice that didn't occur. She resides in North Bend, Ore.
- The Board reinstated the registered nurse license of Bobbie M. Felton (200942261RN) and placed it on probation for 12 months for violating the terms and conditions of a previous Board Order. She resides in Coquille, Ore.
Deschutes County- The Board accepted the Voluntary Surrender of Sue A.
