Written license verifications
If a written Oregon license verification is needed, there is a $30 verification fee. Please send the required verification form (from the State requesting the verification) or send the Board a written request (via fax or email) with the verification information needed (the licensee name and contact information, and the recipient information) and request an electronic invoice to pay online.
NOTE: The Board performs primary source verification of the highest level of education for each of the licensees
As discussed at the November 6, 2015 Board meeting, discipline cases from the last 15 years are included on the Board's website. At their September 9, 2016 meeting the Board updated the website discipline policy that the posting of withdrawn orders will be considered on a case by case basis. Respondents can request a withdrawn order be removed from the Board's website and the Board will consider each case and make an independent decision on each request.
If you’d like more information about a licensee or case, please complete a public records request. There are details of the case and investigation that are confidential. By completing a public records request, the agency will release any public record documents associated with the request.
