OHA accepted a complete notice on October 18th, 2024 and is conducting a comprehensive review of this transaction. The 180-day comprehensive review period is currently tolled pending additional information from the entities. This transaction is also subject to CCO Form A review.

Community Review Board

OHA plans to convene a community review board for this transaction. Community review boards are temporary groups formed solely to provide input to OHA about a particular health care business deal. The community review board has two key roles:
  1. Help OHA understand how a deal could affect people and communities in Oregon.
  2. Make a recommendation about whether the deal should be approved, approved with conditions, or disapproved.

Application Process

OHA is currently accepting community review board applications for this transaction. To apply, please submit an application, conflict of interest form, and demographic form to hcmo.info@oha.oregon.gov by 5 pm on Friday, January 24, 2025. We've included both PDF and Microsoft (MS) Word versions of the forms; you only need to submit one version of each form and can use whatever version is easiest for you.
