Community health improvement plan review, implementation and evaluation; engagement strategies for person- and family-centered health care systems; strategies to address social determinants of health

Community Catalyst’s Center for Consumer Engagement in Health Innovation supports efforts to improve the health of people and communities with complex health and social needs. We amplify the voices of consumers and communities in health care and in health policy. We promote innovation and health care that is person-centered and focuses on the well-being of vulnerable people and communities across the country. The Center builds upon Community Catalyst’s many years of success working with partners in more than 40 states.

The Center’s consulting helps to advance efforts of health plans, hospitals and health systems, and public programs in utilizing consumer engagement to create value by driving better health outcomes and better business results. Our consulting offers a range of consumer engagement-related services, including the following:

  • Customized assistance to clients to help them establish structures and build skills to support the meaningful engagement of patients, family caregivers and/or community residents.
  • Technical assistance on recruiting and working with consumer advisory committees, sustaining participation, and maximizing their effectiveness.
  • Building collaborative relationships with community-based organizations.
  • Integrating evidence-based, self-management programs into care plans for patients with chronic conditions.
  • Assessments for the purpose of planning and quality improvement.
  • Consumer involvement in program design and evaluation.
  • Communication and feedback mechanisms for patients and family caregivers.
  • Strategies to build multi-sectorial alliances to address social determinants of health.
  • Informational webinars and trainings that support consumer engagement and health system transformation efforts.

Our Oregon work is led by Renée Markus Hodin, deputy director of the Center for Consumer Engagement in Health Innovation.
