Behavioral health integration; primary care transformation, including patient-centered primary care homes; value-based payment methods
E. Dawn Creach, MS, is an independent consultant who is passionate about improving health care in Oregon. She brings a diverse set of skills including program design and evaluation, strategic planning and communications, program management, technical writing, stakeholder engagement, facilitation, and consensus building. Dawn’s areas of expertise include behavioral health integration, patient-centered primary care home implementation, payment reform, health policy, and patient experience of care surveys.
Since 2004 Dawn gained this breadth of experience in various settings including five years in research at OHSU, three years in state government, two years leading a statewide child health quality improvement initiative, and three years at an independent physicians association. Throughout her career, she has conducted site visits at more than 100 primary care clinics, and specializes in advancing delivery model innovations including PCPCH and behavioral health integration.
As an independent consultant, Dawn’s current clients include PacificSource Health Plan, Legacy Health, and the Children’s Health Alliance providing a variety of services including technical assistance, practice assessments, strategic guidance, and program development.
