Primary care transformation, including patient-centered primary care homes
Roberta R. Kaplan, PA, MPH, MS, is a results-oriented health care professional with over 30 years of experience working in the federal, state and private sectors. She has expertise in compliance, quality improvement, research, analysis and evaluation, policy development, project management and clinical practice. Ms. Kaplan can identify, organize, synthesize and analyze the information and data needed to help organizations recognize problems, understand processes and create solutions.
Ms. Kaplan was one of the original site visitors for Oregon’s PCPCH program. During that time, she conducted over 45 site visits. Ms. Kaplan evaluated program performance and outcome measures, wrote comprehensive reports, and monitored recommended corrective action. She also coached clinic staff and developed and implemented extensive pre-visit, on-site and post-visit processes.
In 2014, Ms. Kaplan started InnovaHealth Innovations, a health care consulting company whose goal is to help medical practices and patient-centered medical homes improve the quality of care of their patients. This is accomplished by delivering expert guidance to identify and implement evidence-based strategies and best practices for patient care. Ms. Kaplan currently provides consulting services to the Health Resources and Services Administration’s Bureau of Primary Health Care, where she conducts operational site and technical assistance visits to federally qualified health centers.
Clients include HRSA, Management Solutions Consulting Group, KEPRO, NACHC, OHA and URAC.
