Dr. Roger Kathol, an ABMS Board Certified internist and psychiatrist, with 22 years of experience in the practical application of integrated clinical practice at the University of Iowa, recognized a need to share his knowledge and expertise with other health care providers by incorporating an integrated care consulting company, Cartesian Solutions, Inc.™ Initially, consultants participating on Cartesian Solutions, Inc.™ expert teams assisted hospitals and clinics develop value-added medical setting-based inpatient and outpatient medical and behavioral health programs to augment their care of complex comorbid medical and surgical patients.
Between 2000 and 2003, Dr. Kathol was hired by Blue Cross Blue Shield of Minnesota to utilize his expertise in integration at the health plan level. He assisted the company review its “carved-in” behavioral health business practices and adjust its service support work processes. After completing this work, Cartesian Solutions, Inc.™ expanded its consultation offerings to include integration assistance for health plans, businesses, and government agencies. By this time Dr. Kathol had developed a robust understanding of the interaction between payment procedures and the ability to deliver sustainable integrated behavioral health care in the medical sector, where the majority of those with behavioral conditions seek treatment and significant “excess” medical service use occurs.
Since 2003, Cartesian Solutions, Inc.™ has completed hundreds of consultations to its targeted clients. Clients are now moving past relatively easy integration steps, such as collocating providers and subsidizing behavioral health services in outpatient clinics. Rather, they are now starting to build systemwide value-added integrated services, to revamp payment approaches for medical and behavioral health benefits, and to develop integrated cross-disciplinary case management programs.
Based on client requests, Cartesian Solutions, Inc.™ pulls together expert teams of clinical and non-clinical co-consultant professionals capable of providing customized solutions for diverse clients. All consultations center on the development of programs or procedures related to improving health and cost in those with concurrent medical and behavioral health challenges in medical settings. They also include the way in which primary behavioral health patients needing specialty sector services fit into the total health picture.
