Public health integration

Public health integration refers to the integration of population-based, preventive health services and system changes into the CCOs’ implementation of the coordinated care model that supports a shared goal of population health. The OHA Transformation Center partners with the OHA Public Health Division to coordinate technical assistance for CCOs. The OHA Public Health Division is part of Oregon’s public health system and supports the triple aim of better health, better care and lower costs by collecting and monitoring data on the health of Oregonians and using that data to implement community health strategies that work.
