Engagement strategies for person- and family-centered health care systems

Engagement, broadly defined, is an active partnership among individuals, families, health care clinicians, staff, and leaders to improve the health of individuals and communities, and to improve the delivery of health care. Collaborative individual and family engagement is an important strategy for developing a person and family-centered system of care. Through the systematic application of the principles, strategies and practical tools of person and family-centered care, CCOs can increase meaningful engagement of individuals and families in their own health and in the programs and policies governing health care services.
