Coordinated Care Organization (CCO) Plans

About CCO Plans and the Oregon Health Plan (OHP)

OHP's local health plans are coordinated care organizations or CCOs. Except for extra services and pharmacies, all CCOs offer the same OHP benefits. You choose your CCO when you apply for OHP.

  • If you are an OHP member and don't have a CCO, you can choose one in your dashboard at ONE.Oregon.gov. You can also call Client Services (800-273-0557).
  • If you don't know if you have a CCO, you can ask Client Services.

How to Compare Plans

 Filter by your county's name

  • Click the filter icon at the top of the table below, then click "Counties."
  • Type in the name of your county, then click "Add filter." Do not add punctuation or the word "county" when typing the name.
  • CCOs that serve your county will have your county listed in the "Counties" field.

 Make sure the CCO serves your ZIP code

  • "All ZIP codes" means the CCO serves the whole county.
  • If certain ZIP codes are listed for your county, this means the CCO serves only those ZIP codes.

Review CCO materials

  • To find CCO providers, view the CCO’s Provider Directory. You can search for specific providers by name, specialty or location.
  • View the CCO’s Member Handbook. You can learn about the services the CCO provides and how to access them.
  • To print information about a CCO, choose the printable PDF file in your language.
