FHIAP Direct Deposit Information for Group Members
 
To sign up for direct deposit for your subsidy check, download and complete the Direct Deposit Authorization Form. Note: Do not fill out this form until notified by FHIAP that you have been approved and can now sign up for direct deposit. Failure to follow FHIAP's steps will result in your direct deposit form being returned.
 
Please DO NOT fax this form; original signatures must be on file with the Department of Administrative Services. Do not send the form to FHIAP. This is handled by a different state agency.

Be sure to have your financial institution complete and sign Section C of the Authorization form before mailing it to DAS.
 
Please note: The Social Security Number (SSN) as well as the payee name and address needs to match the current account holder and payee for FHIAP. If it submitted in another family members name DAS will not be able to complete your request.
 
How long will this take?
Once the form is received by the Department of Administrative Services, setting up the connection between your financial institution and DAS takes nine banking days. Meanwhile, you will continue to receive your subsidy payments by warrant/check, as usual. DAS will notify you of any problems.

Direct deposit will not necessarily speed up the process of receiving your subsidy reimbursement. It takes two banking days from the date your payment is processed for the money to reach your account.

Change Of Address
Change of address information for direct deposit
 
If you change your address, you must tell FHIAP. FHIAP can submit your new address through the normal vendor change process. DAS does not handle your subsidy checks. You can contact FHIAP at 888-564-9669.
 
To change a mailing address, send this information to FHIAP. Include the following:
  • Your Social Security Number
  • Your name (print)
  • Your new mailing address, and
  • Your signature. The signature must be the person who signed the original authorization form. For security reasons, this request has to be an original; copies or faxes are not accepted.
