These include Tomando Control de su Salud, the Spanish-language version of Living Well, the Diabetes Self-Management Program, and the Positive Self-Management Program (PSMP) for people living with HIV/AIDS.

About Stanford Self-Management Workshops

Living Well with Chronic Conditions is the same as the Chronic Disease Self-Management Program (CDSMP) developed by Stanford University. For more information about Stanford programs, please see our Self-Management Program Fact Sheet or visit the Stanford Patient Education Research Center website.

Some Stanford programs focus on people living with specific conditions like diabetes or HIV, but all cover the following topics.

  • Managing symptoms
  • Medication "how-to"
  • Working with your health care team
  • Setting weekly goals
  • Effective problem solving
  • Better communication
  • How to relax
  • Handling difficult emotions
  • Tips for eating well
  • Safe, easy exercise

Program Length

A six-week program that meets 2½ hours each week. (Note: Positive Self-Management runs for seven weeks.)

Who Attends

People with different chronic conditions and their support people (family or friends).


Two trained leaders at each location. At least one of them has a chronic condition.


Some sites may charge a small fee to cover the cost of the course materials, which includes a book.
