About the Self-Management Network
Network Vision
To support the organizations that affect change within our communities through expansion of sustainable evidence based self-management programs that support better health, better care and lower costs.
Network Purpose
- Promote successful statewide expansion, implementation, and coordination of evidence-based chronic disease self-management programs
- Facilitate communication and collaboration between organizations that support self-management programs in Oregon
- Provide a venue for addressing challenges and opportunities that affect all programs and partners
- Support the sustainability of programs statewide by enhancing local ownership of programs and sharing knowledge and resources
Membership
The Network is open to all organizations providing
evidence-based self-management programs in Oregon and to other partners interested in self-management, including representatives from health systems, providers, insurers, and community-based organizations. We also encourage participation from self-management program leaders. If you are interested in joining a workgroup or the general Network calls, please contact us.

Steering Committee
The Network Steering Committee is made up of dedicated volunteers who:
- Provide leadership and organizational sustainability for the Self-Management Network, ensuring representation of Self-Management Network members
- With Network members, determine direction and priorities for the Self-Management Network
- Coordinate activities across workgroups
- Identify opportunities to expand and support self-management programs statewide and determine strategies to utilize such opportunities as they arise.

Quarterly Network Teleconference
The Network meets regularly (via phone) and in person at the Oregon Place Matters Conference. Each meeting includes a presentation on a timely topic, as well as workgroup updates, announcements, and time for networking.

Self-Management Network Workgroups
The CCO/Finance workgroup and Quality Assurance, Fidelity and Measures workgroups are currently on hiatus.
Participant Recruitment and Leader Engagement Workgroup
This workgroup focuses on identifying tools and best practices in Oregon to keep trained leaders engaged and increase outreach to future participants.
