In State Transfers
For transferring cases, contacts, LTBI and B-waivers within the state of Oregon, please see the instructions for Transfers Within Oregon (pdf).
For transferring contacts within the state of Oregon, complete the Contact Transfer Form (pdf) and send to the appropriate local health department. If receiving this contact transfer form, please record all evaluation and treatment information on the form and send back to the referring local health department at end of follow-up (please send a copy to the State as well).
Transferring TB Cases and Contacts to Other States, U.S. Territories and Countries
To transfer TB cases or contacts out of Oregon but within the U.S., complete the Interjurisdictional TB Notification From (IJN) and fax to the receiving jurisdiction. Directions on completing the form are on the NTCA website. Information for each state can be found here: State TB Control Programs. A note should be left in Orpheus regarding the case or contact transfer.
Ask the receiving jurisdiction to complete the Interjurisdictional TB Notification (IJN) Follow-Up Form and return it to you and/or call the receiving jurisdiction directly to obtain information about the outcome of the referral. Information on the outcome of the referral should be entered in Orpheus for the referred case or contact.
Attempts should be made to transfer all TB cases on treatment who are leaving the U.S. utilizing Cure TB. A note should be left in Orpheus regarding the international transfer of a case.
Contacts that are moving to or reside in Canada, Mexico or a U.S. Territory (e.g. Marshall Islands, Chuuk) should be transferred. Use Cure TB to transfer contacts to Mexico. Information for transferring contacts to U.S. Territories: American Samoa, Federated States of Micronesia, Guam, Northern Mariana Islands, Puerto Rico, Republic of Marshall Islands, Republic of Palau, U.S. Virgin Islands.

If you receive a transfer to
your jurisdiction, you must inform the referring jurisdiction about the outcome. Complete and forward the Interjurisdictional TB Notification Follow-Up Form
to the referring jurisdiction.
