In 2022, the Oregon Health Authority Transformation Center hosted a monthly virtual learning collaborative to support coordinated care organizations (CCOs) and other organizations that provide care coordination services to Oregon Health Plan (OHP) members. The learning collaborative focused on understanding and meeting CCO 2.0 care coordination requirements and facilitated sharing of care coordination and intensive care coordination best practices.
