The Office of Community Health and Engagement (OCHE)

Our Community Partner Network

Formerly the Community Partner Outreach Program, OCHE has built a one-of-a-kind network to serve every county in Oregon. OCHE has written agreements with local trusted organizations who:

  • Provide culturally and linguistically responsive outreach, health coverage application and enrollment assistance and system navigation.
  • Focus on supporting communities facing disproportionate barriers to health coverage and health-related resources.

Our network includes approximately 350 organizations and around 1,900 community partners, also known as assisters, across Oregon.

  • Contracted organizations include physical care and mental health providers, faith-based organizations, non-profits, schools and more.
  • Community partners, or assisters, are people affiliated with a contracted organization. They are trained and certified to help people understand health coverage options and help them apply. This help is free.

To address the social determinants of health, community partners may also help connect people with resources such as food pantries, SNAP, rent assistance, energy assistance, unemployment benefits, job resources and more as capacity allows. 

Our Work 

OCHE’s regional and statewide team provides support to our diverse network of community partners. We:

  • Train, certify and provide ongoing support in your community.
  • Hold monthly community partner collaborative meetings across the state to share updates about OHP and other health coverage, outreach strategies, and community resources. 
  • Serve as the bridge between the community and the state of Oregon to support OHP enrollment and health care system improvement.

When funding is available, we may offer grants to community partner organizations.
