About the Student Health Survey
The SHS is a comprehensive, school-based, anonymous, and voluntary survey administered to students in 6th, 8th, and 11th grades across Oregon. It is a key part of statewide efforts to help local schools and communities ensure that all Oregon youth are healthy and successful learners. The survey provides schools, communities, and state agencies with valid, population-level data about youth health and well-being, including mental and behavioral health, sexual and reproductive health risks, substance use, school climate, and protective factors.
Collecting demographic information—including sexual orientation and gender identity (SOGI)—helps identify health disparities among youth. This data supports targeted prevention efforts and the development of resources for populations at higher risk.
The SHS is designed to address:
- Student health and safety
- Student mental and behavioral health
- School climate and culture
